• Unite the Union LGBT+ Reporting Form

Unite Equalities is seeking to understand any issues or concerns that may have arisen in your workplace following the recent release of the draft EHRC Code of Practice. We would welcome your feedback and invite you to use this tool to share any experiences, concerns, or impacts you may have encountered, despite this code being restricted to service users and associations. Your responses will help us better understand the experiences of members and identify where further support, guidance, or action may be needed. Please also indicate, in response to the question below, whether you are happy for the information you provide to be shared with your Unite Regional Officer, who may be able to offer further advice and support.
  • To help us better understand the issues being raised, we would like to ask for the name of your workplace and your job title. This information will help Unite identify the sectors and roles where these issues may be occurring and ensure that we can better understand the experiences of members. Providing this information is entirely optional and you can choose whether or not to complete these fields. However, sharing these details will help Unite develop a clearer picture of the challenges being experienced and consider where further support may be needed. Unless you select the option below to provide consent, any information you submit will only be accessible to Frazer Lake, National Officer for LGBT+ Members.
  • If you would like any support with these issues, if you have not raised them already, you can fill in your personal details below. This is entirely optional to fill in and is your choice, but it will then allow us to locate your membership to offer support. Unless you select the option below, your personal information will only be seen by Frazer Lake, the National Officer for LGBT+ Members - however, if you are happy for us to share this with your Regional Officer/Regional Equalities Officer for support, this option is below for you. There is also a separate option to check if you are happy for an anonymised version of your issue to be shared with the National LGBT+ Committee. Again, this is entirely optional and any information that would be shared would have all personal and identifying information removed prior to sharing.
  • Are you happy for this information to be shared with your Regional Officer/Regional Equalities Officer to support you further?*
  • .*
  • An anonymised copy of the issue you have raised will be shared with the National LGBT+ Committee. This will help the Committee better understand the experiences and challenges affecting our LGBT+ members and identify where further support or action may be needed. For absolute clarity, no personal information or any details that could identify you will be shared. Only anonymised information about the issue itself will be provided

  • Thank you for taking the time to fill in our feedback form. If you have any further questions, please feel free to contact us on UniteLGBTPlus@unitetheunion.org

    • The information you provide on this survey will be used by Unite in the following ways:

      •             if you choose to provide this information, your name, job title and workplace will be used by Unite to try and identify if these issues are happening in a certain sector. This will not be seen by anyone outside of Unite.

      •             information about workplace issues will be used to assess if there are any patterns in sectors/workplaces.

      •             if you provide your email address and/or phone number, and your consent for us to contact you, Unite will use this to provide information to you and to organise support around the points raised and other issues that might be affecting or useful to you.

      •             Your responses will be provided to the National Office for LGBT+ Members. If you choose to give your consent your details and responses will be passed on to your Regional Officer/Regional Equalities Officer for support.

      •             The Union will share fully anonymised information  with the National LGBT+ Committee so they can review the scale of issues that are being raised. No personal information will be shared with any members of the committee.
