      This survey is for workers of the employer, workplace and/or sector identified on page one of the survey form. Any information which might identify you will be kept confidential by Unite the Union ("Unite") and will not be shared with your management or anyone else outside Unite. The information you provide on this survey will be used by Unite in the following ways:

      • your name, job title and workplace will be used by Unite to map the workforce. Mapping information may be shared with Unite's elected workplace representative(s) to provide information to you about the issues raised and to organise and recruit Unite members;
      • information about workplace issues will be used to assess what's important to the workforce and to organise support or build a campaign around this;
      • if you provide your email address and/or phone number, Unite will use this to provide information to you and to organise support around the points raised and other issues that might be affecting or useful to you.

Unite contacts members by post, telephone, SMS or email about issues relevant to their union membership in accordance with our privacy policy. Members can change how we communicate with you generally by visiting the MyUnite website or by contacting your Regional Office to do this for you.

      The information provided by you will be stored securely and electronic records will be retained by Unite for the duration of any organising campaign.
